Specialized trainings are on going for different colleagues in oncology as follows:
Drs-2 Drs have completed thei r super-speciality in Onco –Sugery and Gynae Onco.
3Drs are still on training 1-Medical Oncologist,1Paediatric Oncology and 1 interventional radiology, 1 will be going for Nuclear Medicine training in Feb 2015.
2 Radiation Technologists are at Ocean Road undertaking their studies.
1 Pathologist who is in the department of Histopathology since 2007.
1 Medical Oncologist who is the head of department has his training in Super specialist in that field in Italy completed in 2008 , and he is in the department since 2009.
1 Nuclear Medicine Physician completed his studies in 2011.
1 Medical Physicist completed his studies in 2012 together with 1 Radiation Technologists and 2 Nuclear Medicine Technologists.
4 Nurses were trained in Onco-nursing 3 in Italy for 6 months each, and 1 in South Africa for 4 months.
Apart from that we have in house training in the department,we trained 26 Drs in the Region surrounding Bugando or Served by Bugando as a tertiary Hospital in September 2013.
We also had a training for Nurses and Pharmaceutical Technicians 35 in total in June 2014.
In house training is ongoing due to staff turnover for training or other reasons.
Community
During different outreach campaigns especially screening,the community is reached and awareness on the risks,prevention and early detection are being done.
Also this exercise is being done at the department on daily basis during treatment schedules,special activity eg Pink and Red Ribbon months,also when patients are waiting to be seen in the clinics through posters and leaflets.
EARLY DETECTION AND SCREENING.
This is done in House,Facility and Outrech based.
- In house is meant for staff we expected 300+ but we screened 96.
- Facility means Clients coming for screening every Friday of every week voluntarily –We have screened 356 women.
- Outreach Screening which is funded by Italians Collaborators we were able to visit 8 districts and screened 3000 women, we are currently analyzing the results.
TREATMENT AND CARE.
This activity started way back in 2009,Patients were mobilized from different wards to oncology ward-J4.Both adults and Childres.The number of patients treated has exponentially increased as shown on the graph below.
PALLIATIVE CARE.
This is again divided in Facility and community.
- Facility
Offered in the Hospital we created a palliative team which is currently not functioning well,but in a month at the oncology Department we can see up to 10 cases.
- Community
We are using the existing Home based care for HIV to reach our clients ,with this community Palliative care a number of chronic Diseases are inclused,HIV/AIDS,Diabetes Mellitus,Hypertenstions and theirComplications eg Stroke,Muscular Disoders etc.
The Oncology department at Bugando spearheaded to form a lake Zone Palliative initiative in collaboration with Tearfund Foundation,the Draft of the document was submitted to the Ministry for approval.
SUPPORTIVE CARE.
Peculiarity for the treatment of cancer render special attention during the course of treatment,areas of challenges are Long duration of treatment, detachement from family,transportation Burden,Drop out from school,Divorce etc.

INTERNAL MEDICINE DEPARTMENT:
Internal medicine is the largest department in Bugando medical Centre. The department it serves both in and patients in specialized and super specialized services. In order to provide an excellent services to the patients the department is organized into different units according to specialization with the special medical ward s for both male and female gender.
Bugando Medical Centre provides Private and VIP services to their patients who need that kind of treatment. Patient in the private and VIP wards usually gets high quality services at the fast truck mode being served by specialists and super specialists daily. The Private Wing has 38 beds including a 5 beds High Dependence Unit (HDU) with 24 hours critical nursing services.
The VIP wing has 18 rooms and each room has 2 beds for the patient and his or her relative. The rooms are furnished with a TV, air condition and a free WiFi. Medical services include daily medical specialist round, and fast tracked laboratory and imaging services. The hospital has well structured medical wards for other general patients who get admitted in the hospital.
NEPHROLOGY UNIT

Nephrology unit is the fastest growing unit in terms of having exponential raise in number of patients in the department of internal medicine. We have highly motivated and experienced nephrologists who take care both in patients and outpatients in the nephrology unit and the entire hospital at large. We attend more than 300 out patients weekly and more than 3 patients get admitted daily in the nephrology unit. Most of the patent in the unit present with end stage renal disease requiring renal replacement therapy.
We have very good and well equipped dialysis unit in the Lake zone with more than 20 haemodialysis machines (Fresenius Brand from Germany).We perform more than 1200 haemodialysis sessions monthly. We have experienced and well trained dialysis nurses in the unit who provide services tirelessly. Both patient with acute and chronic kidney disease are taken care of in the unit.We also have the plan to start Kidney transplant.

Research and consultancies are also done in the cardiology unit also in collaboration with other specialized hospitals outside the country.
CARDIOLOGY UNIT
Cardiology is another unit in the department of internal medicine. We have highly motivated cardiologists and other cardiac specialized physicians. We provide high quality cardiac services to both in patients and outpatients. More than 300 cardiac patients attend the cardiac clinic weekly and around 3 to 5 cardiac patients are admitted daily in the cardiac unit. Heart failure from different causes being the most common cause of admission in the unit.

The unit well developed with a lot of improvement in patient’s care. High specialized equipment are available in the unit. We have highly experienced specialists in the fields who can performs most of the procedures in the unit. The unit take care of both in and out patients. We attend more than 300 out patients weekly and about 3 patients are admitted in the unit daily. The most common cause of admission is schistosoma related upper gastrointestinal bleeding.

Physiotherapy department is one amongst the earliest department at Bugando Medical Centre. It was established 1971 at the first opening of Bugando Medical Centre.
Since then, Physiotherapy department has been growing in numbers of staffing, quality of service delivery and expanding in delivering physiotherapy services to inpatients, outpatients and community at large.

In other words this department is aiming on delivering standard and excellent Physiotherapy and rehabilitative services at this consultant & teaching medical facility.

The basic objective of our services at the hospital intends to minimize the number days of patient admission particularly for those with physical related impairment as well as to reduce number of disabilities within the society at large.
The core objective of physiotherapy is to improve physical function, reduce pain, and prevent disability through therapeutic exercise, manual therapy, education, and other modalities. This includes promoting physical strength, flexibility, endurance, and mobility, as well as enhancing cardiovascular and respiratory functions, and improving balance and coordination. The roles and functions of physiotherapy include:
- Assessment: Physiotherapists conduct a thorough evaluation of patients' physical conditions, including, range of motion, strength, and function, to create a customized treatment plan.
- Diagnosis: Physiotherapists use assessment outcomes to diagnose and develop treatment plans unique to the patient's health condition.
- Treatment: Physiotherapists provide treatment plans based on the diagnosis. This includes manual therapy, therapeutic exercise, Ergonomic advice, Proper posture and patient education.
- Education: A significant role of physiotherapy is educating patients and developing home exercise programs to promote self-management, prevent re-injury, and improve general health.

OCCUPATIONAL THERAPY UNIT
Occupational Therapy (OT) is the holistic and client centered treatment of the Physical, Cognitive, Psychiatric and Psychological Conditions by the use of meaningful activities in order to help the patient reach the maximum level of function and independence in their everyday life activities
Occupational therapy unit started in 2012 with ANNA a foreigner volunteer as its first Occupational therapist, later in 2016 the first indigenous full time occupational therapist was employed. Since then, the hospital has hired two more occupational therapist making a total of three occupational therapists up to date.
Role of occupational therapy
- Occupational therapist help in adaptation of physical environment at work and at home to overcome barriers that a person face during performance of activities of daily living
- Occupational therapist use standardized/non standardized tools to assess, diagnose and plan treatment for specific conditions which affect participation in activities of daily living.

Description
EMERGENCY MEDICINE DEPARTMENT
Emergency Medicine Department was formally established in September 2012. The department currently handles a load of an average of 120 patients per day (including outpatients and high acuity patients). It is staffed by 3 qualified, locally trained Emergency Physicians supported by nurses, medical officers and medical attendants; all trained in recognizing and treating emergencies, who provide 24 hours services. The department also offers ambulance services with qualified staff to handle emergencies enroute.
The department has recently undergone a major rehabilitation and expansion project which has enabled the bed capacity to increase from 12 to 24 (including infant warmers for neonates). The expansion project has enabled the department to have 2 emergency operating rooms, 24 hour pharmacy services, 24 hours laboratory services and facilities for registration and payment of the various services offered.
The department is equipped with various equipment to aid resuscitation and stabilization of patients such as; patient monitors, ventilator, defibrillator, ECG machine and ultrasound machine. All these assist the timely provision of care to patients arriving at EMD. The department also has the ability to take care of mass casualty events in collaboration with staff from other departments.
All patients, regardless of their ability to pay, are attended at the EMD and other formalities follow after stabilization of the patient. The hospital accepts many insurance providers and has contracts with a number of institutions both private and governmental.

- It has a capacity of fifty beds , admitting both males and females, however the inpatients ranges from forty to eighty at a time with total number of outpatients per day thirty to ninety.A

Description
INTENSIVE CARE UNIT (ICU) DEPARTMENT
Intensive care is the service provided for patients with potentially recoverable conditions who can benefit from detailed observation and invasive treatment. ICU department deals with for the provision of care to critically ill patients. It provides intensive and specialized medical and nursing care, an enhanced capacity for patient monitoring, and multiple modalities of physiologic and mechanical organ support to sustain life during a period of life-threatening organ system insufficiency. Our ICU is the largest ICU for capacity in Lake Zone caring critically ill patients around Lake Zone and the nearby regions.
The ICU at Bugando Medical Centre in the year started in the year 1996 as a ward under with four beds with only one nurse. At his time there was only one mechanical ventilator to support patients. In 2005, the ICU extended from 4 bed capacities to 12 beds and in this year there were 6 nurses available to care of ICU patients (1:6, nurse to patient ratio) and one doctor to oversee the unit. From this time, the hospital continued to hire nurses who underwent different form of training including on job training and by the year 2021 we had a nurse to patient ratio of 1:2-3.
In 2021, the ICU was made to be a standalone department in the hospital. And by this year,2021 the hospital received support from the government to renovate the intensive care unit ward where by the hospital manage to renovate the ward and also it expanded the ICU from the existed 12 beds to 29 beds ICU capacity. From 2022, the ICU started operating as surgical and medical ICU separately. Together with this expansion of the ICU the numbers of nurses have also increased (with the nurse to patient ratio of 1:2 for every shift). Other medical staff number and qualifications has also increased as for nowadays we have anaesthesiologists, intensivist, ICU physician, and medical doctors readily available all the time to care of critically ill patients. The number of equipment to monitor and support patients has also increased and for now the unit has more advance equipment to provide care for critically patients.
ACHIEVEMENTS
- We have achieved a lot of in terms of increasing in capacity from 4 beds o 29 beds capacity ICU and we have a separate medical and surgical ICU.
- We have good number of advanced functional equipment for monitoring and providing mechanical support patients.
- We have managed to have qualified nurses including ICU specialist nurses and doctors to take care of critically ill patients in our ICU. More than 90% of our ICU staffs have been trained for Basic Life Support and resuscitation.
- We have succeeded to support our nurses to acquire training in critical care for both short and long duration courses.
- Our ICU has been providing attachment for both and nurses and doctors who need to learn critical care management.
FUTURE PLANS
In the next five years our plans are:
- To offer critical care training for both nurses and doctors
- To continue extending our services to more specialized ICU care such as renal and cardiac ICU.

Introduction
Dermatology is a branch of science dealing with diagnosis and treatment of diseases of the skin, the hair, the nail and the genitals. A dermatologists treats your child’s chronic fungal infection of the scalp, also treats your teenagers disturbing acne, deals with your chronic itching skin and takes care of many more symptoms on your skin , hair, nails and genitalia.
History
Our history goes back to the year 2015, where we started as a Unit of Internal medicine department .In 2015 dermatology unit was started by Dr Nohrasco Mang’ondi. Since then, more specialists have been trained in this field, and currently, we have 5 trained dermatologists. In July 2022 dermatology unit became an independent department breaking from internal medicine department. From July 2022 to present the head of department i s Dr Nelly Mwageni.
Activities
1. Attending all patients with disorders of the skin, hair and nails.
2. Attending all patients with sexually transmitted infections (STIs).
3. Performing Dermatological surgical procedures such as; skin tumour excision, keloid excision, skin grafting, wound dressing, genital wart cauterization etc.
4. We offer inpatient services to serious patients.
5. We provide specialized care for people living with albinism (PLWA) and xeroderma pigmentosum patients
